To reserve your Telehealth (Phone Call) Re-Certification Consultation with the physician, payment is required at the time of scheduling unless alternative arrangements have been approved by our office in advance.

Please confirm your appointment when contacted by our office via text, email, or phone.

If you need to cancel, reschedule, or are running late, please notify our office as soon as possible. Patients who arrive more than 5 minutes late without notice may be considered a missed appointment and may need to be rescheduled.

For Telehealth (Phone Call) appointments, please be available at your scheduled appointment time and answer calls from our office and physician. If we are unable to reach you for your scheduled phone appointment, your appointment may be considered missed and rescheduling may be required.
